legal purpose.

I will keep the parts,
kits, and
finished products out of the reach of children.

I will properly label the finished
products and secure them against use by children and those
unfamiliar with safety procedures.

I will not use any LASER products
indoors or in confined spaces where reflection hazards exist, or
where others may be injured.

I am legally responsible for the use of these components, improper use of
these components, or their end products.

I will not use LASER parts,
kits, or
components to intentionally injure or disable any human being or
living creature.

I am legally responsible for any injury to anyone resulting from the use of
or assembly of these components or their finished products.

I
accept LASER parts and components as COMPONENTS for integration in a system of
MY
OWN design and will hold the SELLER LEGALLY HARMLESS from any and
all LIABILITY.

I understand that LASERS and LASER
components are dangerous to handle, and affirm that I am aware of
safe handling and assembly procedures.

I acknowledge that directing a LASER
beam of any power level at an aircraft or moving vehicle is a
FEDERAL OFFENSE and a SERIOUS FELONY in ALL STATES for which I can
be arrested and imprisoned.

I agree NOT to use my finished LASER
project for "sky-pointing" or "astronomy instruction" in areas where
the beam might interfere with aircraft traffic.

I acknowledge that LASERS over 50
milliwatts may damage eyesight if the beam or its reflection is
viewed directly.
-
I acknowledge my understanding that
high-power LASERS – particularly BLUE 445nm and 405nm can
INSTANTANEOUSLY and IRREVERSIBLY damage my vision, or the eyesight
of others – even at distances of 300 yards or more.

I agree to ALWAYS wear eye
protection, and have others in the area wear eye protection when I
am using LASERS.

I agree that Awesome LASERS, LLC, its
principals, officers, agents, and resellers / affiliates are not responsible for
damages, injuries, deaths, accidents, inconveniences, or other ill
effects arising from my use of or my inability to properly use or
assemble these LASERS or LASER components.

I
know the electronic safety precautions necessary with these devices and
therefore I accept it AS-IS as an EXPERIMENTAL COMPONENT without
express or implied warranty.
I acknowledge that electronic parts such as diodes and driver boards
purchased from Awesome LASERS are
NOT RETURNABLE
once the sealed ESD bag / container
/ package is opened.

Awesome LASERS'
Laser Diodes, Modules, Pills, and
Hosts are designed solely
as OEM components for incorporation into the customer's end
products. Therefore, these
kits, when assembled
DO NOT
comply with the appropriate requirements of FDA 21CFR, section 1040.10 and
1040.11

I affirm that
when assembled,
these devices may not legally be sold in the USA and other
countries.
Awesome LASERS cannot sell
me, and has not sold me an assembled "kit". I also
acknowledge that I cannot legally sell finished LASERS from "kits"
that I assemble.
I AFFIRM THAT I HAVE READ AND FULLY UNDERSTAND ALL OF THE
ABOVE, AND AGREE WITH ALL STIPULATIONS SET FORTH HEREIN. I AM BOUND
